(Two Copies) • Original PAYMENT RECEIPT (Two Copies) • Original Caste Certificate + 1 True Photocopy (Xerox) • Original MARK SHEET-CLASS 12 + 1 True Photocopy (Xerox) • Original ADMIT CARD- CLASS 10 + 1 True Photocopy (Xerox) • Original SCHOOL LEAVING CERTIFICATE issued by the institution from where he/she has passed class-12 examination + 1 True Photocopy (Xerox) • Original EWS (Economically weaker section) CERTIFICATE or BPL Ration Card.
